Scheduling an Appointment

Direct Access

In Wisconsin, there is direct access to physical therapy, which means you can come directly to a PT without having to see your physician first. At Health In Balance Physical Therapy we work with you and your physician. If you do not have a physician, and it is indicated that you need one, we will assist in finding an appropriate one, usually within the week. Your insurance may require a doctor's prescription before they will pay any benefits.

If you do not have a prescription and are unsure if physical therapy is appropriate, we offer a free 15-minute injury screen. We will assess your condition, determine your needs and begin a dialogue with your doctor to obtain a prescription for physical therapy. Please note that some insurance companies do require a prescription in order to cover physical therapy services.

Insurance
Please call our office so we can verify we are in network with your insurance. If you don't have insurance coverage you may be seen as part of our Prompt Pay program. Our Prompt Pay rates are $160 for the initial visit, $120 for a 45-minute follow-up visit or $80 for a half-hour visit. You will be required to pay for your appointments prior to your session. We accept personal checks, VISA, MasterCard, and Discover. We do offer Prompt Pay packages which offer a discount on the rates.

Workers Comp

We are a skilled Workers Compensation provider and work with many WC insurance companies. We are able to get you scheduled within 24 hours of calling. Please call us to schedule.
