In Wisconsin, there is direct access to physical therapy, which means you can come directly to a PT without having to see your physician first. At Health In Balance Physical Therapy we work with you and your physician. If you do not have a physician, and it is indicated that you need one, we will assist in finding an appropriate one, usually within the week. Your insurance may require a doctor's prescription before they will pay any benefits.
If you do not have a prescription and are unsure if physical therapy is appropriate, we offer a free 15-minute injury screen. We will assess your condition, determine your needs and begin a dialogue with your doctor to obtain a prescription for physical therapy.
If you have already scheduled an appointment with us you may print out the forms below to complete and bring with you to your first appointment. If you have not made an appointment, please call us at 262.236.0176 or use our online Request an Appointment form.
Please call our office so we can verify your insurance. If you don't have insurance coverage you may be seen as part of our Prompt Pay program. Our Prompt Pay rates are $150 for the initial visit, $140/hour for follow-up visits or $70 for a half-hour visit . You will be required to pay for your appointments prior to your session. We accept cash, personal checks, VISA and MasterCard.
In a few days you will have your first physical therapy appointment. Please arrive 15 minutes early for your appointment and wear loose-fitting clothing. Please fill out the registration form provided above, and bring them with you to your evaluation. Your therapist will be asking you many questions regarding your problem. She will also take range of motion measurements, test your strength, and perform special tests as it relates to your diagnosis. The therapist will gather as much information from you through the evaluation process to formulate an individualized therapy plan. Your point of view and goals for recovery are important. You will also be given exercises to work on at home which are imperative to your recovery.
It is normal to experience some soreness, increased pain, or a change in your pain location when first starting therapy. As you get stronger this will diminish and you will be able to tolerate more advanced exercises. You will also notice life tasks will be less painful and easier for you. Your physical therapist will continually reassess your progress toward your treatment goals and will work with you and other health professionals to plan for your discharge from physical therapy.
